The city of Ottawa has started the H1N1 vaccine clinics - You can get more information on all clinic locations as well as answers to basic questions HERE

You are required to fill out a consent form at the clinic - You can print and prefill the form HERE

 

Walter Baker clinics will be held on October 28 ,29 and November 23, 24 from 2:30pm until 8:30pm

 

West end clinics from Oct 26 - Nov 27 are:

City of Ottawa —Administrative Building 100 Constellation, Nepean [map]	Monday to Friday: 5 to 9 pm Saturday and Sunday: 9 am to 3 pm
Kanata Recreation Complex 100 Walter Baker, Kanata [map]	Monday to Friday: 2:30 to 8:30 pm Saturday and Sunday: 9 am to 3 pm
